Disease Management

are you looking for a doctor to treat your chronic illness and surgery if necessary?

Do you suffer from a chronic disease such as cancer, ALS (Lou Gehrig’s Disease), arthritis, Alzheimer’s disease, dementia, asthma, Chronic Obstructive Pulmonary Disease (COPD), Crohn’s Disease, Ulcerative Colitis, Inflammatory Bowel Disease, Irritable Bowl Syndrome (IBS) or Cystic Fibrosis? We can help you! Dr. Sosa and our primary care clinical staff are experienced in helping patients manage their disease while improving their quality of life. We offer screenings, check-ups, monitoring and treatment coordination with our patient’s care team even if those healthcare providers are out of county, out of state or even international.

We work with our patients to manage their condition and minimize the effects of their disease. We also monitor medications, and we may make recommendations to adjust a dosage or type of medication as we see progress or improvement in one’s health. Typically, a patient living with a chronic disease has more than one doctor treating them for the condition. Disease management involves a team approach, not a single care provider working with their patient, instead it’s a team of health care experts. Our care team will communicate with the other providers involved in a patient’s care to protect the patient’s quality of life and all options are considered to manage their disease.

As part of a disease treatment plan a patient may be prescribed anti-inflammatory drugs, antibiotics to treat infections, water pills, vaccines as a preventative and possibly a surgery. We work with our patients every step of the way to ensure their care and quality of life does not diminish.
